India’s approach to early childhood care is undergoing a significant transformation. No longer limited to routine vaccinations, the government is now set to provide continuous, structured oversight for every child from birth until the age of three (36 months). This ambitious initiative aims to identify health risks early, ensuring that newborns and toddlers receive timely interventions and specialized care when needed.

Comprehensive Monitoring from Day One

Under the newly launched Integrated Child Health Program, introduced by Union Health Minister Jagat Prakash Nadda, the focus extends far beyond immunization. Health workers will now track each child’s physical and developmental milestones regularly. If a baby is born with low birth weight, prematurely, or shows signs of delayed growth—whether physical or mental—they will be immediately classified as high-risk. These children will then receive targeted medical attention and follow-up care.

Infants who have spent time in Special Newborn Care Units (SNCUs) will also be flagged for intensive monitoring. They will undergo frequent check-ups and, if necessary, be referred to specialist doctors for further evaluation and treatment.

Home-Based Guidance for Families

ASHA workers will play a central role in this expanded system by visiting families at home. Their guidance will cover several critical aspects of early childhood development:

  • The importance of exclusive breastfeeding and why it should be the only source of nutrition for the first six months.
  • The benefits of skin-to-skin contact, also known as Kangaroo Mother Care, for both full-term and premature babies.
  • How talking, playing, and building emotional bonds with the child supports cognitive and social growth.
  • Practical advice on maintaining a clean and safe home environment.
  • Age-appropriate nutrition, play, and mental stimulation strategies after the first six weeks.

Recognizing Danger Signs Early

The new guidelines also equip families and health workers with a clear list of warning signs that require immediate medical attention. These danger signs include:

  • Refusal to feed or difficulty breastfeeding.
  • Breathing problems or rapid, labored breathing.
  • Repeated seizures or convulsions.
  • High fever that does not subside.
  • Persistent diarrhea or blood in the stool.
  • Lethargy, unconsciousness, or unusual drowsiness.
  • Severe chest retractions (the chest sinking in noticeably).

In any of these cases, the child must be rushed to a hospital without delay.

Climate and Heat Protection for the First Time

For the first time, these guidelines explicitly link climate change to child health. ASHA workers will advise families on how to protect young children from extreme heat and air pollution. Key recommendations include:

  • Avoid taking babies outdoors during peak afternoon sun.
  • Dress children in light, breathable cotton clothing.
  • Increase breastfeeding frequency during hot weather to prevent dehydration.
  • Minimize exposure to polluted air by keeping windows closed during high-smog periods.

Screen Time: A Strong Warning for Parents

The government has issued a clear advisory regarding screen exposure for children under three years old. Parents are urged to keep mobile phones, tablets, and other digital screens away from toddlers. Instead, families should encourage face-to-face interaction, storytelling, and physical play. These activities are far more beneficial for a child’s developing brain than passive screen time.

Digital Tracking for Every Newborn

To ensure no child slips through the cracks, every newborn’s details will be recorded on the Janani Portal and the Shishav App. High-risk children will be digitally flagged, allowing ASHA workers, Auxiliary Nurse Midwives (ANMs), Anganwadi workers, and doctors to coordinate their care seamlessly. This digital infrastructure will enable regular monitoring and timely referrals, creating a safety net for the most vulnerable infants and toddlers.
